 • NOTE: Everyone who climbs or belays must have a waiver form signed. Anyone under the age of 18 must have a waiver signed by a parent or legal guardian. If the waiver form is not signed in the gym, it must be notarized or you must have a legible photo copy of the driver's license of the person who signed the waiver. New waiver forms are required yearly.
